How to refer
Referral is easy
Diagnostics
When referring a patient for diagnostic imaging, such as MRI, X-ray, ultrasound or CT scan, you have the following options:
Online - please click here for our online referral form.
By post - either print our PDF referral form, or contact your local centre to be sent pre-printed forms, and post to the relevant centre (click here for centre addresses).
By fax - obtain a referral form as above and fax to your local centre (click here for centre fax numbers)
Once we have your completed form, we will contact your patient to arrange a convenient time for them, and send you the report within 48 hours of appointment.
Specialist Consultants/ Physiotherapists
For onward referral to a further specialist please see our list of practicing consultants/ health professionals, then contact us via phone/ post or fax and one of our receptionists will either book your patient in for an appointment, or advise you of the Consultant's secretary's contact details.
